    CONSENT TO THE PROCESSING OF DATA

    1. As defined in Article 4.7 of the EU Regulation 2016/679 of 27 April 2016 (GDPR), the role of the controller of personal data submitted via the Research Teams Database in Digital Medicine registration form is assigned to the National Information Processing Institute (OPI PIB), with its registered office in Warsaw at Al. Niepodległości 188B. The controller can be contacted by email at med@opi.org.pl 
    2. The controller has appointed the Data Protection Officer, who can be contacted by email at iod@opi.org.pl for any issues regarding personal data and exercising the rights of data subjects.
    3. Personal data will be used to create, maintain and develop a database that collects information on individuals and teams of individuals (research teams) who conduct healthcare research and projects driven by digital technologies and artificial intelligence. Personal data that is collected in the database will be processed to contact and establish collaboration between said individuals and teams of individuals, share their information, experiences, knowledge and skills, promote the results of their research and development projects, and assist entities that seek support and expertise in specific fields. Personal data will also be processed to develop analyses and reports concerning healthcare research and development projects that are driven by digital technologies and artificial intelligence for the purposes established by the controller and entities that operate in that field.  
    4. Personal data will be processed based on the legal basis specified in Article 6(1)(e) of the GDPR, which allows personal data to be processed if it is necessary for tasks that are conducted in the public interest, as stipulated in the Polish Act on Research Institutes and the OPI PIB’s statute. Tasks conducted in the public interest include the provision of services related to the collection and development of aggregate, cross-sectional, and synthetic information concerning scientific research and development work, technology transfer, innovation, and education. Personal data that has been submitted for contact purposes or provided voluntarily in the registration form will be processed according to Article 6(1)(a) of the GDPR, i.e. on the basis of consent given in the form of declaration or explicit action consisting in completing the fields in the registration form that are marked as optional. 
    5. Recipients of personal data include entities that seek support and expert knowledge in healthcare driven by digital technologies and artificial intelligence to conduct their activities, scientific research, and development work, as well as service providers contracted by the data controller to fulfil the objectives in section 3 above, particularly those offering IT, consulting, and legal services. 
    6. Personal data will be processed until the purpose for which it is processed expires. Personal data that is processed based on the necessity to fulfil a task in the public interest will be processed until the data subject objects to the processing of their data for reasons related to their particular situation unless the controller specifies valid, legally justified grounds for data processing that override the interests, rights and liberties of the data subject, or grounds for establishing, asserting or defending claims. If any claims are filed, personal data will be processed until all legal remedies available to the parties in this regard have been exhausted. Personal data that is processed based on consent given by data subjects will be processed until such consent is withdrawn.  
    7. Data subjects have the right to:  
      1. access their personal data (Article 15 of the GDPR),
      2. rectify their personal data (Article 16 of the GDPR)
      3. restrict the processing of their personal data in the circumstances and according to the rules specified in Article 18 of the GDPR, as well as to delete their personal data as per Article 17 of the GDPR,
      4. object to the processing of their personal data on grounds relating to their particular situation (Article 21(1) of the GDPR). 
    8. If personal data is processed based on consent given by data subjects, data subjects have the right to withdraw their consent at any time without having to provide a reason of withdrawal and without affecting the lawfulness of processing based on consent before its withdrawal. Consents can be withdrawn by sending an e‑mail to the controller at med@opi.org.pl or by submitting a request by mail to the controller’s registered office address. 
    9. Data subjects have the right to submit complaints pertaining to the processing of their personal data by the controller to the President of the Polish Personal Data Protection Office (address: ul. Stawki 2, 00-193 Warszawa), 
    10. Providing personal data is voluntary and optional, but necessary for the referred to above tasks to be fulfilled.   
    11. Personal data will not be processed for the purposes of automatic decision making, including profiling. 
    12. Personal data will not be transferred to any third countries or international organisations.
